Family TIES Youth Transition Jamboree

Project Description:
The Youth Transition Jamboree was an opportunity for youth, their parents and families, educators and health care professionals to learn about how they can help youth with special health care needs (YSHCN). When YSHCN leave the school system, they find their transition planning did little to address their healthcare needs. Their pediatric health services and providers are no longer available to them, and their dependency on parents and other adults leave them unprepared to advocate for themselves. The Youth Transition Jamboree presented information about all areas of adult life and the services available to support YSHCN and/or disabilities as they make that transition, emphasizing the need for the youth to understand their health care needs and to take over the management of them as much as possible. Participants gained a new understanding of how and why the health needs of youth can and should be addressed right along with their vocational and educational needs to assure a successful transition from pediatric to adult health care, school to work, and home to community. The role of the NCED in planning this event was significant, as well as the participation. Mary Bryant participated in an expert panel on self-advocacy and Scott Harrington presented on Customized Employment to young adults and their family members. Another event is planned for February 2011.
