Project Description:
The New Hampshire Bureau of Elderly and Adult Services in partnership with UNH IOD, NH Alzheimer?s Association, Dartmouth Centers for Health and Aging and local ServiceLink Resource Centers, is enhancing the existing network of community supports to better service individuals in the early stages of Alzheimer?s disease and related disorders (ADRD) and their caregivers. The objectives are to 1) develop a strategic outreach plan; 2) reach out to the medical community and integrate them into the network of community supports; 3) develop, enhance and facilitate specific trainings for clinicians, care providers, individuals with ADRD and their family caregivers and embed these trainings within the ServiceLink network; 5)provide consumer-directed caregivers supports; 6) evaluate the impact of the trainings and services; and 7) issue a final report. The expected outcomes of the project are: 1) 100 physicians and medical professional will receive training in ADRD and supporting individuals and caregivers as they are diagnosed; 2) 120 individuals and family caregivers will receive training on legal and competency issues; 3) 200 individuals and family caregivers will be trained in curriculum for Alzheimer-proofing your home; 4) 100 individuals and caregivers will be trained in Taking Control curriculum; 5) 100 individuals and families will receive support and educational materials; 6) 12 Caregiver Specialists will gain skills to help individuals and caregivers with early stage ADRD.
