Project Description:
Increase the capacity of transitioning students, families, Santa Fe community and the Santa Fe Public Schools to support the successful transition of students to adult life using:
*Quality of Life Interviews
*PATH (Planning Alternate Tomorrows with Hope) futures planning
*parent groups
*community mapping
*employer and community resource recognition
*and networking
Family members, teachers and students are supported to learn to complete Quality of Life interviewing and PATH planning.
