Project Description:
SCOPE OF WORK
Outcome 1: Increase capacity of DOH-DD Waiver providers to evaluate communication needs of participants of the Waiver who have vision impairment, hearing impairment and/or deaf-blindness.
Outcome 2: Increase capacity of DOH-DD Waiver providers to establish appropriate and effective communication strategies to utilize with participants of the Waiver who have vision impairment, hearing impairment or deaf-blindness.
Activities:
1) Contractor to provide 2 regional, 1 1/2 day trainings on communication assessment and strategies to teams of DOH providers servicing 20 individuals who have sensory impairment. DOH providers will then go back to their communities and complete communication assessments on individuals who have sensory impairment. Document attendance at conference-specialty training in sensory impairment and communication strategies for DOH providers, completions of communication assessments for 20 individuals.
2) Contractor to provide individualized technical assistance to 6 participants of the DD Waiver, and their provider teams. TA to include: Review of communication assessment and strategies, On-site consultation or use of Telehealth services to discuss alternative strategies. Report recommendations for 6 individuals, modification of communication assessment and strategies.
