Corazon de la Familia is a home-visiting program providing early intervention, prevention, and treatment services to children under age 5 and their families.
Project goals/objectives/activities:
The goal of the program is to assist families in optimizing the growth and development of their high-risk infants, toddlers, and preschoolers, particularly children with special health care needs. All interventions designed for young children are developed with parent/caregiver input and provided at a level of understanding appropriate to the family. Particular program expertise exists in the provision of specialty services to very young children from Latino families living in poverty communities and experiencing pervasive and multiple stressors. In-home services are designed to meet family needs, and are based on recommendations of the referring agency. Intensity and duration of services is dependent upon levels of identified need. Our approach is relationship-based; all services are designed to be child-centered, family-focused and culturally appropriate. The home visitation protocol has been developed to assist families in the following areas: planning activities appropriate for child's age and level of development; screening and education on development, behavior, health, and environmental safety; managing difficult behaviors in young children; family support; parent education and anticipatory guidance; crisis intervention; nutritional education; health monitoring; coordination with other agencies and primary care providers to improve medical care of children. The program has extensive experience in service provision and in working with multiple agencies to coordinate services for families, in linking families with services, in tracking families to measure outcomes, and in gathering data.
