"Family One-Stop"
Abstract:
In partnership with the Institute on Disability at the University of New Hampshire and the New Hampshire Developmental Disabilities Council, this project will build community bridges to improve career and recreational opportunities, financial independence, and family support options for individuals with developmental disabilities and their families in the 15 rural eastern Monadnock Region towns. The services of the One-Stop Center will focus on three areas: (1) Individual and family directed planning for community participation; (2) Transition planning and career development; and (3) Individual and family economic development. A minimum of 50 families will receive direct services annually as the project demonstrates improved community engagement and individual and family outcomes. The partnerships from the planning grant will be expanded to include the Aging and Disability Resource Centers, career centers, businesses, and others.
