[email protected] Function: Information Development and DisseminationThe focus of the CSHCN (Children with Special Health Care Needs) Medical Home Partnerships for Children and Their Families project is to promote quality health and developmental care for all children in Washington state, especially those with special health care needs or children at risk for developmental or health concerns. Quality of care is promoted through active partnerships with primary health care providers, families and community service providers. Current project activities include the following:1. Implementation of quality care guidelines for monitoring low birth weight infants' health and development through adolescence, using the Critical Elements of Care for the Low Birth Weight NICU Graduate guidelines and the supplement care guidelines for the extremely low birth weight infants 2. For parents, implementation of materials that partner with the above guidelines, including Watching Your Low Birth Weight Child Grow: Tips for Parents and Was Your Baby Born Early? Was Your Baby Born Small? 3. Technical assistance and training to facilitate community partnerships with primary care providers including the distribution of Child Health Notes, a one-page newsletter offering current information on the identification and management of special needs conditions4. Promotion of medical homes for children with special health care needs by maintaining the state Medical Home Leadership Network. 5. Facilitation of local and state efforts to pilot Bright Futures materials in Washington.
