Evolving Care Coordination Benefits Under Medicaid

Project Description:
There is widespread agreement among providers, parents, and health policy analysts in New York State and nationwide that the present system of financing and organization of services for children with special health care needs (CSHCN) does little to enhance their quality of life or that of their families. There is clear recognition that there are barriers to care that need to be removed. To date, there has been no effort to create a consensus on how to optimize access to care. And yet the consequences of well-coordinated health care are considered by experts to be vital for children with serious chronic illnesses or disabilities, not just to stabilize their condition, but to advance their growth and development so they may be productive members of their families and contributors to their communities.What conditions will promote growth and development for CSHCN? To what extent do public and private insurance and payment systems to providers discourage access to health care for CSHCN? What kinds of care-coordination programs have emerged for special needs populations, under the aegis of Medicaid? Answers to these questions will generate a better plan in New York State for delivery of health services for CSHCN.
