Project Description:
Alaska's Real Choice Systems Change project will design and implement effective and enduring improvements to the community long-term support system that will enable children and adults with disabilities or long-term illness to be present and participating in their communities. The project goals are focused on making improvements in the following areas: 1) overall systems management; 2) access to services and supports; 3) adequacy of services and systems; 4) quality of services received; and 5) value of services received. Project goals include: 1) Ensure that a planning, capacity building, monitoring and advocacy structure is in place that will result in real choice systems change; 2) Integrate self-determined service delivery into current service delivery systems; and 3) Improve access to services through systems reform and the development, implementation and evaluation of consumer-driven care coordination/case management systems.
