Project Description:
The CDD, in collaboration with its state government based partner, the Office of Disability and Health, will sustain a strong and innovative program of health promotion, research, and capacity-building including: ? Implementation of two proven health promotion interventions that have been pilot-tested in the state, The Take Charge Challenge and Living Well with a Disability, with three targeted populations: Centers for Independent Living, seniors and youth 14 to 21 years of age; ? Innovative dissemination of training packages for these interventions, including web-based courses; ? Continued collection, analysis and dissemination of disability data from the BRFSS through a series of policy briefing papers and technical assistance workshops; ? Collection, analysis and dissemination of existing disability data through creation of a database merging disability data from administrative datasets in state agencies in cooperation with the New Mexico Developmental Disabilities Planning Council; and ? Capacity building efforts within and outside New Mexico to ensure that the health promotion interventions, BRFSS data and data from existing administrative datasets are used by community-based organizations within the state, Level I, II and III CDC-funded programs, and other programs across the nation both during and after the grant period. The activities described in this proposal have been developed with the active cooperation and advice of members of the New Mexico Disability and Health Advisory Council (DHAC), and are consistent with and fulfill the New Mexico State Plan for Promoting the Health of People with Disabilities, which focuses on access, health promotion, disability data, physical activity/nutrition and women?s health.
