Product Description:

This Case Management course prepares BHA-I trainees to provide culturally grounded, community-based support within the context of the Wind River Indian Reservation. Trainees learn the foundations of case management; how to identify and connect clients to resources; how to make and follow up on referrals; how to document professionally and protect confidentiality; and how to support medication management, group participation, crisis response, discharge planning, and continuity of care.
