Product Description:

The following case studies highlight schools' and families' experiences in the seamless transition pilot. For the first six case studies, from different pilot locations, we use the metaphor of carving a path to illustrate a team's journey through rough and smooth terrain. Every team member on each journey has different perceptions of the experience while helping each other along the trek. This second case shows the importance of strengthening trust and cohesion in interagency transition teams.
