Product Description:

A presentation on the Transition to Adulthood Council (TAC) which supports healthcare transition- focused clinical programs at the Insitute on Development & Disability at OHSU through direct collaboration with youth with disabilities. TAC members build community, develop leadership, mentorship and advocacy skills, and experience and explore their own power as changemakers, with the goal of improving provider support and health literacy in the area of healthcare transition.
