Background: The PreventionLink project, led by the Prince George's County Health Department (PGCHD), was implementing innovative interventions to address high blood pressure and high blood cholesterol in Southern Maryland, between 2019-2023. Objective: To promote hypertension management in low-income populations, by using a hypertension self-management app with remote blood pressure monitoring.
