Project Description:
The Institute for Community Inclusion at the University of Massachusetts Boston in partnership with a broad-based consortium consisting of the local educational agency, public agencies, community organizations, advocacy groups, youth with ID/DD, and family members, will over the course of this five year project, support the development and implementation of a plan to transform transition services in the city of Lawrence, Massachusetts for youth with ID/DD to a sustainable model that results in improved employment and post-secondary education outcomes and long-term career success. The project will focus on a traditionally underserved community with a high immigrant population and extensive socio-economic challenges, guided by the principles of cultural competency and humility. The interrelated goals of the project are: (1) to improve collaboration among local partners, leading to (2) improved transition processes and experiences for youth and families and (3) improved transition to work outcomes for youth with ID/DD in Lawrence. The objectives are: 1. Establish a multi-stakeholder consortium, the Lawrence Partnership for Transition to Employment (LPTE); 2. Conduct a community landscape analysis; 3. Work with the LPTE to develop a Community Collaboration Employment Transition Plan; 4. Implement the Community Collaboration Employment Transition Plan. The proposed outcomes are: a) Improved and sustained collaboration among partners; b) Improved transition process for students with ID/DD, that are individually driven aligned with the principles of self-determination ; c) Improved post-secondary and employment outcomes for youth. Products will include policy and practice materials in support of the implementation of the Community Collaboration Employment Plan.
