Project Description:
The Nevada Center for Excellence in Disabilities (NCED) will provide trainings on transition topics, with an emphasis on healthcare transition to Children and Youth with Special Health Care Needs (CYSHCN) and their families. The NCED will begin the creation of Nevada specific transition information to distribute among the trainees. Need: Schools and transition programs for youth focus on employment and independent living skills, but little is being done to help families with health care transition. Goal 1: The Nevada Center for Excellence in Disabilities will provide trainings on pediatric to adult health care transition, medical home, and the Nevada Medical Home Portal to professionals serving families of children and youth with special health care needs and developmental disabilities. Benefit: Families will be educated and will have better youth health care transition outcomes.
