Project Description:
One major problem in complex clinical care is fragmentation. Children receive health care from multiple clinicians who work in different programs, organizations and systems and who maintain their own care plans and records. Children attend preschools or schools that generate additional plans and records. An integrated care plan is needed to improve organization and communication. This project launches a multi-stage research program designed to develop, implement, and evaluate a model integrated health care plan. We will use this initial funding to develop an interface and basic design for the integrated care plan. Parents of children with prolonged neonatal hospitalizations or genetic/chromosomal disorders will provide input to the design of the interface. A larger and more diverse group of parents plus clinicians and administrators will evaluate the prototype using qualitative and quantitative methods. We will subsequently seek extramural funding to build a fully functional web portal with a mobile application. We will then test whether children who use the integrated health care plan show reduced unmet needs, decreased redundancies, and improved clinical and functional outcomes in comparison children who use the current system.
