Assuring Better Child Health and Development (ABCD III)
Project Description:
The goal of this project is to improve the communication infrastructure between primary care settings and the Early Intervention system. The project is a multi-agency effort between the Oklahoma Health Care Authority who is the primary grantee, the Oklahoma State Department of Health and the OUHSC Child Study Center.
Project Purpose: Improve communication between primary care settings and Early Intervention to assure eligible children receive services as early as possible.
Strategy: Creation and deployment of a web portal to connect PCPs to Early Intervention and community partners. Participating PCPs will send referrals to SoonerStart via the web portal and receive referral outcomes through the portal.
