Project Description:
The goal of this project is to improve the communication infrastructure between primary care settings and the Early Intervention system. The project is a multi-agency effort between the Oklahoma Health Care Authority who is the primary grantee, the Oklahoma State Department of Health and the OUHSC Child Study Center. Project Purpose: Improve communication between primary care settings and Early Intervention to assure eligible children receive services as early as possible. Strategy: Creation and deployment of a web portal to connect PCPs to Early Intervention and community partners. Participating PCPs will send referrals to SoonerStart via the web portal and receive referral outcomes through the portal. Funded jointly by the Commonwealth Fund and Oklahoma Health Care Authority (SoonerCare Choice Medicaid) Project administration from Child Study Center, OUHSC Three Years: Year One - Planning - completed October 2010 Year Two - Implementation in Four Counties - 10/2010 to 10/2011 Year Three - Statewide Rollout - 10/2011 to 10/2012 Four Counties for Pilot: Canadian County Garfield County Pottawatomie County Tulsa County Key Players-State and local community level teams comprised of: OUHSC Child Study Center/OHCA (project management and Practice Facilitators) Sooner SUCCESS (care coordination and local capacity building) OSDH/OSDE SoonerStart Early Intervention (services for children 0-3) OSDH Child Guidance (services for children 0-8) Oklahoma Family Network (family-to-family networking) PCPs (pediatricians, family physicians, nurse practitioners, PAs)
