Project Description:
Health Care Program for Children with Special Health Care Needs (Title V) is an annual contract for shared personnel working on collaborative projects which include a Social Worker, Occupational Therapist, Nurse, and Developmental Pediatrician.
Personnel work with the fourteen (14) HCP Regional Offices, HCP State office staff, HCP designated community partners, and providers throughout the state of Colorado to: 1. Participate on the program's multi-disciplinary team for policy and program development and provide consultation from the perspective of the discipline of occupational/physical therapy and social work/mental health and to represent their respective field of study. 2. Promote and facilitate local needs assessment activities for occupational therapy, physical therapy and social work/mental health services through the fourteen (14) HCP regional offices and county nursing services, to analyze the local community occupational therapy, physical therapy, and social work/mental health needs assessment data from all fourteen (14) HCP regional offices, using state and national data, and to develop a collaborative (inter and intra agency) technical assistance/training plan on an annual basis, to occupational therapy, physical therapy, social work training, technical assistance and quality assurance to the staff in the fourteen (14) HCP regional offices and county nursing services, including occupational/physical therapy and social work regional coordinators. 3. Disseminate, promote, explain standards of care to the general public, HCP regional office teams, county nursing services and/or specific providers and to be responsible for the development of state guidelines and standards of care for populations of children with special health care needs, particularly those that most relate to the field of occupational therapy, physical therapy, and social work/mental health, through the coordination of interagency advisories, educational/training offerings, care coordination systems, and task forces. 4. Initiate and manage projects and grant proposals that will help coordinate programs and agencies efforts that will help build statewide systems of services and supports and will answer questions about standards and best practices. 5. Assure the implementation of all program policies and contract agreements, including budget, to assigned regional office(s) through consultation, training and technical assistance. 6. Assure ongoing program development and quality assurance of the assigned regional office(s) through training and consultation, i.e., community-based planning components that address the six national performance measures for CSHCN, management practices, communication, family-centered care, customer service, etc. 7. Review, solicit statewide input, and make recommendations for the Program's occupational therapy, physical therapy, and social work/mental health policy development and revisions. 8. Maintain communications with the multidisciplinary team via monthly reports and participation at scheduled meetings of the Consultation Team, MCH Consultants, manager of the HCP Consultation Team, biannual HCP Team Leader meetings, and other meetings as determined to be necessary.
The social work consultant is responsible for the coordination of the Developmental & Evaluation (D&E) Clinic Program, which includes the following work activities: 1. Evaluate and manage the contracts with Medicaid and Colorado Department of Education (CDE). 2. Keep current records/documentation on clinic locations, clinic coordinators, clinic status, clinic data, and clinic policies and procedures and make them available to HCP, CDE, Medicaid and other interested community members. 3. Assure that quality standards are being met and communicate, as needed this information to HCP, CDE, and Medicaid. 4. Pursue continued physician training to increase community capacity. 5. Provide ongoing technical assistance and consultation to clinic coordinators, clinic fiscal agents, community providers, and others concerning the administration, structure, and function of the D&E clinics. 6. Collect, analyze, and provide clinic program data to local clinics and state offices at HCP, CDE, and Medicaid. 7. Submit a Six Month Progress Report by May 15 of every year, and a Final Progress Report by January 25 of every year to HCP and CDE. 8. Assist local clinics to obtain Medicaid, Medicaid HMO, HMO, CHP+, and private insurance reimbursement. 9. Explore the feasibility of telemedicine and tele-education for the D&E clinic program. 10. Conduct minimum of two statewide conference calls per year for physicians, clinic coordinators, parents, fiscal agents, and other interested community members. 11. Cultivate referrals and assure an appropriate and consistent referral process across the state through public awareness, trainings, and ongoing community contacts. 12. Maintain and coordinate the D&E Advisory Board by holding biannual meetings and ongoing communications with board members as needed in-between meetings.
This contract also provides a developmental pediatrician to: 1. Provide medical consultation to the Director of the Health Care Program for Children with Special Needs (HCP) and the State D&E Clinic Coordinator on training and educational programs for medical providers working with the D&E clinics, and D&E program policies. Provide ongoing consultation, technical assistance and mentoring to participating physicians. 2. Provide leadership for the HCP Medical Home Initiative by making contacts with the Medical Home Advisory Board; participate on the Medical Home Strategic Planning Committee; provide consultation, training and technical assistance to the 14 HCP Regional Offices as needed to assist in the development of their CSHCN plan related to National Performance Measure #2; and provide ongoing consultation to the HCP Program Director, with regards to the HCP Medical Home Initiative. 3. Assist with the coordination of the Medical Home Initiative and grant writing with other programs within the Division of Prevention and Intervention Services for Children and Youth. 4.Act as the physician liaison to the Part C Public Awareness project. 5. Coordinate and manage grants and projects as appropriate. 6. Increase exposure of the Medical Home concept through personal contact with community physicians and pediatric trainees and through publications. 7. Participate in other awareness and training activities that relate to the Medical Home Initiative. 8. Relay information about any educational opportunities available through JFK Partners or other LEND programs, that might be applicable to MCH local and state staff.
A Parent Consultant through Family Voices is included to: 1. Provide consultation to the Colorado Medical Home Initiative and the Medical Home Learning Collaborative through attendance at meetings. 2. Provide consultation and data to the interagency Measuring and Monitoring Data Group and attend monthly meetings. 3. Provide information regarding the activities of Family Voices. 4. Coordinate with the HCP Family Consultants for the Family to Family Health Information Network Project.
JFK Partners provides coordination of the Maternal Child Health Continuing Education Series. 1. Plan and implement 4 training events designed for MCH state and local managers and staff incorporating the use of videoconferencing for 3 of the events to provide access to those who live and work in many of the rural areas of the state.
2. Oversee and organize the 4 training events including flyer development, arrangements with the videoconference sites, speaker logistics, online registration, and management of participant handouts. 3. Debrief by phone with the Maternal Child Health Director and State office staff after each event to improve future events. 4. Relay information about any opportunities about any education opportunities available through JFK Partners or other LEND Programs, that might be applicable to MCH local and state staff.
