Quality Assurance and Improvement, Technical Assistance, and Oversight under the Money Follows the Person (MFP) Rebalancing Demonstration
Project Description:
Technical assistance will encompass a range of activities that address the components of transition programs, strategies to ?right size? institutional capacity and other issues identified through the needs assessment information and ongoing communications with each grantee. General issues facing Grantees may include:
? Identification of an array of community supports
? Identification of other evidence-based practices
? Implementation strategies that insure that practices are implemented in a sustainable fashion
? Cross-system planning using quantitative planning methods
? Work with stakeholder groups (making sure individuals and family members are active participants in the design, promotion, delivery, and evaluation of the MFP initiative)
? Development of infrastructure to support policy and planning
? Identifying and implementing sustainable financing strategies to support new service models
? Evaluating the fidelity with which new service models are implemented
? Workforce issues, including recruiting and retaining qualified workers and training
? Development of performance indicators
? Insuring the availability of data to support planning and policy
It is anticipated that substantive technical assistance will be required by MFP Grantees and their stakeholders around the following key priority topics:
? Reimbursement rates for community-based long-term care providers
? Recruiting, training, setting workforce expectations for and retaining transition coordinators
? Identifying effective ways to assist individuals who are transitioning to secure decent, affordable MFP-compliant housing
? Peer-to-peer learning modalities for older adults and persons with disabilities
? Linking the money follows the person initiative to ongoing federal and state mental health transformation efforts
? Having an appropriate array of clinical services and supports in the community for individuals with serious mental illnesses
? Ensuring that there are effective crisis response/crisis management/clinical services delivery networks in place to prevent the reoccurrence of institutionalization for the MR/DD population
? Pinpointing existing policies and practices that pose barriers to the successful transition of individuals from institutional to home and community-based services.
? Designing payment systems that support the delivery of necessary services and supports on behalf of individuals who have challenging conditions, so that they can be supported in MFP compliant living arrangements
? Effective management of the global developmental disabilities budget to support the shift of dollars from institutional to community services
? Revamping service planning processes to ensure that they address the full range of services and supports necessary to accomplish transition to the community and embrace person-centered planning principles and service design
