Project Description:
The purpose of the Nursing Transition Project was to support consumers who are in nursing homes to transition to an alternate setting, either their home or assisted living. The project began in three primary communities: Lincoln, Omaha, and Norfolk. The Norfolk office took the lead in the project. Currently it has been broadened to be statewide. The eight Area Offices of Aging (AAAs) are responsible for its implementation. Each AAA was provided with resources to implement the project. There are two components of the project: 1) implementation of an Awareness Campaign (including TV adds, billboards, etc.,) and 2) implementation of procedures to help facilitate the transition from the nursing homes to alternative settings.
