Product Description:
The Supports Intensity Scale is an assessment tool that evaluates practical support requirements of a person with an intellectual disability to be a participating member in his or her community. Available in print and electronic formats, SIS consists of an 8-page Interview and profile form that tests support needs in 87 areas, and a 128-page User's Manual. No other existing instrument provides a more thorough and comprehensive assessment of supports required by persons with development disabilities. The SIS measures support requirements in 57 life activities and 28 behavioral and medical areas through a one-on-one interview process with the person being evaluated, and friends and family. Life activities measured include the areas of home living, community living, lifelong learning, employment, health and safety, social activities, and protection and advocacy (optional section). The Scale ranks each activity measured according to frequency (none, at least once a month), amount (none, less than 30 minutes), and type of support (monitoring, verbal gesturing). Finally, a Supports Intensity Level is determined based on the Total Support Needs Index, which is a standard score generated from scores on all the items tested by the Scale. A set, including the manual (128 pp) and 25 interview forms (8 pp), is available for $125 from the AAMR. Phone: 301-604-1340 or e-mail: [email protected]
