Senior Project Coordinator for Person Centered Support Coordination

Job Description

The IHD is seeking an experienced and resourceful leader to become a Senior Project Coordinator for Person Centered Supports with the LifeCourse Nexus. This role is highly collaborative and is a key contributor to the successful development and expansion of the LifeCourse Nexus training and knowledge translation goals specifically focused on the role of support coordination (case-management) for those accessing long-term services and supports. This position will support the integration and expansion of the Charting the LifeCourse framework into support coordination and case management practices and organizational cultures.
