Description
The Freeman Center is an interdisciplinary center created to address the lack of specialized, coordinated, patient-centered adult healthcare services. The health system is one of many systems that is difficult to navigate and access. We have collaborated with LEND to assess and respond to the social determinants of health impacting adults with IDD.
