A Patient Centered Medical Home in an inclusive setting, a collaboration of The Boggs Center on Developmental Disabilities and RWJMS Department of Family Medicine and Community Health, provides primary care to adults with IDD and longitudinal training to family medicine residents. With results from a qualitative study this session will demonstrate the importance of care coordination in enhancing access and education. Dialogue on physician training encouraged
