Fundamentals of Medical Home for CYSHCN and I/DD - Healthy Transitions for Youth with SHCN or DD: Training Module for Professionals

Briefly describe the activity and its purpose.

Fundamentals of Medical Home for Children and Youth with Special Health Care Needs is a self-study, online course offered by the North Dakota Center for Persons with Disabilities at Minot State University. The course has five modules for learners:

  1. Care Coordination
  2. Medical Home
  3. Health Benefits Counseling
  4. Healthy Transitions
  5. State and Local Resources for Children and Youth with Special Health Care Needs and Developmental Disabilities and their Families
This course will acquaint learners with the basic components of medical home systems, how to implement in their respective settings, coordinated care, and healthy transitions for CYSHCN & I/DD. It is based on nationally recognized information and materials developed from a federal grant to the North Dakota Center for Persons with Disabilities.

For the fourth module, Healthy Transitions, trainees should read the attached document ("Health Transitions for Children and Youth with Special Health Care Needs" by Ressler). Then, trainees should review the accompanying PowerPoint presentation ("Health Transitions for Youth with Special Health Care Needs or Developmental Disabilities: Training Module for Professionals" by several NDCPD staff). After reviewing these documents, trainees should take the online quiz on Healthy Transitions.

The transition materials included in this submission were collected as part of a grant from the Maternal and Child Health Bureau, HRSA, grant number R40MC19925. The authors of the transition materials agreed to their inclusion in the toolbox.

What are the expected learning outcomes for trainees?

As a result of participation, trainees will:

  • Assist youth and young adults with finding a healthcare provider and how to access needed health care services including routine and preventive services
  • Provide instruction and education on medication self-management skills for CYSHCN/DD
  • Describe various lifestyle aspects of transitioning from youth to adulthood
  • Describe of self-advocacy, guardianship and advanced directives
