11th Annual Chronic Illness and Disability

Skill preparation and planning for health care transition for children and youth with special health care needs (CYSHCN) is inadequate. Only 41% of families nationwide state their CYSHCN have received the services necessary to make appropriate transitions to adult health care, work, and independence. One of the major problems to receiving this care is the lack of a physician and other health care workforce trained in providing the services necessary to make the health care transition. There is a strong need on the part of physicians and all health care providers to have the knowledge and skills to provide services to facilitate successful transition from pediatric to adult-based care.
