Transition Specialist-Northwest Iowa

The University of Iowa Hospitals and Clinics, Center for Disabilities and Development (CDD) is seeking a 100% Behavioral Health Consultant (Transition Specialist).  This position is a specified term with a strong possibility of being extended.  Successful candidates must be able to maintain a home office and travel to the following northwest Iowa counties: Lyon, Osceola, Dickinsin, Sioux, O'Brien, Clay, Plymouth, Cherokee, Woodbury, Monona, and Harrison. 

The Transition Specialist facilitates the process for individuals transitioning from a facility to a home or community-based setting of their choice with provision of supports and services that meet those individual's needs and preferences. Transition services include individual assessments, resource counseling, care coordination and processing of typical and non-typical Medicaid authorized services needed to support the participant's needs. Extensive travel throughout Iowa will be required.  State vehicle provided.
