Vanderbilt Kennedy Center (TN UCEDD) Produces Tips and Fact Sheets on a Variety of Disability-related Topics

The following were produced by the Vanderbilt Kennedy Center (TN UCEDD):

  • Training Manual for Creating Care-Coordination Plans for Individuals with Dual Diagnosis - A treatment model integrating health, psychiatric, and behavioral services for people with dual diagnoses transitioning from Middle Tennessee Mental Health Institute (MTMHI) to the community is now available to be downloaded at no cost. The Community Inclusion Project (CIP) produced the manual titled Training Manual for Creating Care-Coordination Plans for Individuals with Dual Diagnosis and coordinated the demonstration project through the Vanderbilt Kennedy Behavior Analysis Clinic. Funding sources include the TN DMRS, TN MHDD, and TN Council on DD. The goal of the manual is to identify which professionals are responsible for services, and how to work within the framework of the policies governing healthcare for people with dual diagnoses. A second goal of the manual is to provide care providers with an understanding of how independent support coordinators, behavior analysts, psychiatrists, and medical professionals can work together to reduce barriers that influence the quality of life for consumers.

  • Tips and Resources fact sheets on a variety of disability-related topics - Tips and Resources fact sheets are tools for individuals and families with and without disabilities, service providers, students and trainees, and anyone interested in learning more about disabilities and disability-related topics. Produced by Vanderbilt Kennedy Center for Excellence Communication and Dissemination services, Tips and Resources are offered on the science of specific disabilities, the effects disabilities might have on the individual and family, and treatment and service options. They also are tools for clinicians and educators who constantly seek service options for the families they work with. Each fact sheet is a single-page, easy-to-hand-out flyer that can be downloaded at no cost.
