Patient and Family Centered Care Coordination: Integrating Care for Children and Youth Across Multiple Systems

This three-part webinar series will provide an overview of the care coordination framework, function, and benefits for children and families across multiple care systems.

The first webinar in the series, Beyond Policy: Implementing Care Coordination in Practice, will be held on March 30, 2015, from 12:00 p.m. to 1:00 p.m. CT. Topics will include the definition of care coordination and its role in meeting the comprehensive needs of children and their families; clinical guidelines, tools, and resources for implementing care coordination (as outlined in the American Academy of Pediatrics' 2014 policy on pediatric care coordination); and examples of care coordination including strategies and outcomes from the perspective of families and clinicians.
