Join our national learning group! This group will meet monthly for the next 12-18 months to develop a network-wide strategy for implementing healthcare transition efforts that include people with disabilities and families.
Sign up by April 17. Those registered will be sent the zoom link to participate in the kick off call on April 24th 3-4:40pm ET along with an agenda and background materials for the call.
Who should join?
We are looking for AUCD network members who have experience implementing healthcare transition initiatives at the local, state or national levels and are interested in sharing their expertise across the network.
Learning Group Goal
Our goal is to engage the diverse parts of the AUCD network in developing a set of strategies, tactics and tips on how to implement healthcare transition efforts that include people with disabilities and families
Getting Started
In preparation for the kick off call, you will be asked to share resources and tools focused on including people with disabilities and families in healthcare transition efforts and how you've used those resources and tools in your work.

Background and Work to Date:
AUCD has been facilitating exploratory conversations on this topic in various venues over the course of the last year including:
- At the June 2018 LEND Directors meeting
- As a driving change session at the 2018 AUCD Conference
- With leadership of the Health and Disability Special Interest Group (SIG)
- With the Council on Leadership and Advocacy (COLA)
- With the Family Discipline Network
- With leadership of the Rural Area Code workgroup
- With leaders from the LEND PWD workgroup
As a result of these conversations, we have learned about the wide range of impressive work happening in the AUCD network, and we have heard about the priorities for programs and centers around this topic. While there were a large number of issues and concerns discussed in these meetings, a focus on the inclusion of families and self-advocates emerged as a top priority.
We would like to include the whole AUCD network in this work including UCEDDs, LENDs, and IDDRCs, Councils, SIGs, and workgroups.
