This webinar began with definitions and models of care coordination and addressed care coordination with military families. Participants learned about work done in Colorado to define and standardize care coordination functions and outcomes; this work may be useful to other states as they design and enact collaborative models of care coordination. In addition, presenters explained the unique challenges for military families with children who need special services, discussed the role of the care coordinator with military families, and reviewed formal and informal supports for building community capacity to support them.

  • Anne F. Farrell, PhD: Associate Professor of Human Development and Family Studies and affiliated faculty at the AJ Pappanikou Center on Developmental Disabilities at the University of Connecticut
