Topic: Key Issues in Healthcare Decision Making and Care at End of Life: How to use person-centered practices to support quality planning with people with critical, chronic and/or terminal illnesses.
Speaker: Leigh Ann Creaney Kingsbury, MPA, Gerontologist
Leigh Ann is one of a small group of professionals actively working on using person-centered practices for planning around healthcare decision making and care at end of life. She was an invited contributor for an expert panel convened to report to Congress; she is the author of People Planning Ahead: A Guide to Communicating Healthcare and End of Life Wishes, published by the AAIDD in December 2008 and several other articles regarding planning and care at end of life.
Webinar Description: As people with intellectual and/or developmental disabilities have more opportunities for decision making and are living to old age; and as family caregivers are aging, the need to think about healthcare decision making, advance directives and wishes about end of life care is rapidly increasing. It is critical as systems and providers of support services that we begin to help people with ID/DD identify surrogate decision makers for healthcare, should one be unable at any point in time to make his/her own decisions. This presentation will discuss some of the key issues we need to be aware of related to decision making and care at end life, including capacity, competency and how to identify surrogate decision makers.
Target Audience: Executive leadership; QDDP's; ISP/IHP writers, person centered plan developers
