Look-Back form for Family Trainees in LEND Clinics

This look-back form is not an evaluation, per se, but rather a tool to guide Family Trainees to reflect on their experience supporting families in the LEND and other clinics where they support families. There are a number of prompts on the look-back form intended to trigger reflections in three areas: (1) Did the trainee relate to families in the clinic as a peer? (2) Did the trainee provide valuable technical assistance to a clinic that could only have been provided by someone with lived experience? (3) Overall, did the trainee have an equal spot "at the table" alongside clinicians in contributing a family perspective?
