Medical Home in Pediatrics: The HOW TO Webinar Series

The American Academy of Pediatrics (AAP) and its National Center for Medical Home Implementation (NCMHI) have hosted a free, four-part webinar series on effectively delivering care through the medical home model. Building on the success of two previous medical home webinar series, the NCMHI provided engaging educational "how to" presentations focused on several important facets of pediatric care delivery, including empowering youth, reducing health disparities, using data to improve quality, and employing the best practices in family-centered care. The webinars featured expert faculty and provide targeted information and tools; they featured new products from the NCMHI, such as updated state profiles in the Medical Home Data Portal, promising practices regarding language access services, and a new family-centered care monograph.

The transition materials included in this submission were collected as part of a grant from the Maternal and Child Health Bureau, HRSA, grant number R40MC19925. The authors of the transition materials agreed to their inclusion in the toolbox.
