Transition Clinic

We have several clinics with model transition clinics and multidisciplinary teams. The cystic fibrosis pediatric and adult clinics meet twice a month for a joint meeting to discuss clients ready to transition to the adult clinic. The adult team then comes to the pediatric clinic on at least 2 occasions to meet with the client/family before transfer. A questionnaire is given to the transitioning clients to assess what areas they still may need information/training in before transfer. In the spina bifida clinic, the multi-disciplinary team meets with clients before their 18th birthday and assesses needs before transfer to adult providers.
