12th Chronic Illness and Disability Conference: Transition from Pediatric to Adult-based Care

This conference provided a state-of-the-art update on issues involved in health care transition for youth and young adults with chronic illness and disability and their families from pediatric to adult based services. It brought together internationally, nationally and locally-recognized clinical, policy, legal, research, advocacy, youth and family experts, making it a unique conference in the breadth and depth of topics addressing all aspects of transition planning.

2011 AUCD Conference

Thank you to everyone who participated in the AUCD 2011 Conference. It was great to see old friends again, and to forge new relationships. 2011 National Transition Conference: College and Careers for Youth with Disabilities

Equitable Transitions to Adulthood: Issues of Cultural and Social Capital

Research demonstrates that cultural and social capital are important levers in both transition education and the pursuit of equity across diverse groups of people. In this presentation, specific types of cultural and social capital relevant to special education was identified, their potential to impact positive postschool outcomes explained, and the implications for research and practice that take into consideration culture and diversity as complex, interactional variables was discussed.

A Collaborative Interagency, Interdisciplinary Approach to Transition from Adolescence to Adulthood

This Webinar will provide an overview of a paper written by AUCD members who seek to engage in conversations about improving interdisciplinary and interagency collaboration in supporting self-determined transition of youth with intellectual and developmental disabilities to adulthood and to community life.

Scaling-Up EnvisionIT: An Evidence-Based Practice to Improve Academic and Transition Outcomes

This webinar introduced the EnvisionIT curriculum, a web-based curriculum that is aligned with the national Common Core State Standards (CCSS), summarized the research methods used to pilot and refine EnvisionIT thus far, and encouraged applications to scale-up EnvisionIT within and across school districts in their state.
