Skill preparation and planning for healthcare transition for children and youth with special healthcare needs (CYSHCN) and their families is inadequate.  One of the major problems to receiving care is the lack of physician workforce trained in providing the services necessary to make the healthcare transition.  This conference provides a state-of-the-art update on issues involved in healthcare transition for youth and young adults with chronic illness and disability and their families from pediatric to adult-based services. Addressing all aspects of transition planning, this conference strengthens the dialogue among healthcare professionals, policy experts, youth and young adults, and family members.

Broadcast Site Registration Form must be returned to Baylor no later than Friday, October 11th.

This broadcast will NOT be archived!
