Description
In this presentation, we describe our process for partnering with Arizona's Medicaid program to create and implement a uniform system for person-centered service planning (PCP) across the Arizona Long Term Care System (ALTCS) Home and Community Based Settings (HCBS) and Managed Care Organizations (MCO). We discuss our findings from the collection of stakeholder input to date, talk about its strengths and limitations, and outline next steps.Back to Session: Poster Symposium: Systems Change
