Description
Transition to Adulthood is a crucial time period for individuals with neurodevelopmental disabilities. One key to successful transition is planning for the switch from a pediatric to adult healthcare provider. This toolkit focuses on self-advocacy, health literacy, and building independence, all of which prepare for medical transition. Much of the information in the toolkit is generalizable to individuals with intellectual disability and complex conditions other than Trisomy 21.Back to Session: Poster Symposium: Person-Family-Professional Partnerships
