Description

Children and Youth with Special Health Care Needs (CYSHCN) in New Mexico (NM) experience barriers accessing health care and support. This initiative focused on identifying how the NM Title V program Children's Medical Services (CMS) and its medical social workers could improve care coordination for CYSHCN. Focus groups in five communities described barriers and suggestions to improve communication, collaboration, and access to information about resources when providing care for CYSHCN.
