Description

Iowa' s Pediatric Integrated Health program provides care coordination and family support to Medicaid eligible children/youth with mental health challenges and their families. This poster will discuss technical assistance activities that impact family and provider engagement and collaboration during implementation of this new integrated care delivery model infused with System of Care values and principles. The program uses Quality Improvement processes to build linkages with community supports and promote provider collaboration.
