Description

This session will provide an overview of strategies that UCEDDs and LENDs have used in five states to increase awareness about health needs of individuals with disabilities in managed care efforts, such as coordinated and integrated care organizations, state health exchanges, and navigator programs. Ideas on care model and delivery, outreach and education, and partner engagement will be shared. This session is sponsored by the AUCD Health & Disability SIG.
