AUCD - Poster Symposium: Systems Change

Session Description
AUCD poster symposia seek to deepen levels of engagement and connection between those interested in common topics. Posters have been hand-selected by a review committee and grouped by room around common themes. Facilitators will open each poster symposia with broad thoughts about the room's theme followed by a brief 2-3 minute presentation by each poster presenter, approximately 15 per room. Attendees will then have time for more in-depth exploration of the information presented and make connections with others in the room. Attendees are attend to select one symposia to attend during each time slot as space allows; pre-registration is not required.

Dian Baker, PhD, MS, MA, RN , Professor School of Nursing , UC Davis at the MIND Institute, UCEDD/LENDThe presentation will: (a) analyze the significant gaps of data related to children with disabilities and how what monitoring exists for safety and effectiveness of school health services, (b) evaluate the strengths and weaknesses in schools? systems of care for children with disabilities, and (c) provide tools for participants to translate research findings into action.
Transforming Systems of Long Term Services and Supports through Person-Centered Thinking and the LifeCourse Framework

The District of Columbia has been working to transform its developmental disabilities system to include an extensive emphasis on person and family centered thinking. Members of the DC Supporting Families Community of Practice team will share their experience infusing person centered thinking and the Supporting Families LifeCourse framework and tools into system change efforts and reflect on the way that this touches people with disabilities and their families.
"Include Me": Evidence-based Research on the Social and Academic Outcomes for an Innovative Teacher Inclusion Mentoring Model

The Pennsylvania Department of Education designated the Arc of PA to design an innovative teacher mentoring initiative. "Include Me" (IM) aimed to include students (Pre-K to High School) with significant disabilities into regular education classrooms in their neighborhood schools. The 6-year IM model in over 100 school districts; 800 classrooms for 700 students demonstrates significant and notable outcomes in effective instructional practices; student progress; & parent-school partnerships.

Statewide model of building capacity and leadership teams for IEP development in Delaware

This provided training for districts on IEP development and coaching to build leadership teams. Supports are developed to increase capacity of building and district level staff to promote self-sustainability. This initiative provided a model to follow up with a more specific training on IEP development for students with behavioral concerns.
Exploring Premature Exit from the Vocational Rehabilitation System

Catherine Ipsen, Ph.D., Rural Director and Director of Employment Research, University of Montana Rural Institute, UCEDDNearly half of all Vocational Rehabilitation (VR) consumers who are evaluated, determined eligible, and begin services, drop out of VR before completing the program. This presentation reports longitudinal research about facilitators and barriers to retention in the VR program as reported by VR consumers.

�It takes a village� resonates with one of the important characteristics of leadership, Encouraging Others to Act. Participating in the NH LEND program and collaborating with the New England Genetic Collaborative (NEGC) on their stakeholder survey, emphasized the importance of group involvement in creating change. This poster presents methods and results from the 2016 NEGC Stakeholder survey with recommendations to enhance the lives of people in the disability community.

Regional Autism Advisory Council of Southwest Ohio (RAAC): Leading, Planning, and Partnering

Steph Weber, PsyD, Psychologist; Training Director, University of Cincinnati UCE, UCEDD/LENDParticipants will learn about unique collaborations in southwest Ohio with county boards of developmental disabilities, provider agencies, school districts, family members, and self-advocates to address the needs of individuals with Autism Spectrum Disorder (ASD). Descriptions of the work related to training, advocacy, and gaps in services and community need will be addressed. Challenges and benefits will be discussed to encourage replication and further advance community partnerships.

CAPTAIN is an exemplary model of intentionally affecting broad systems' level change by identifying/engaging key stakeholders, and leveraging their existing infrastructures/resources for maximum effectiveness. What began as the vision of a few state leadership members to institute a statewide standardized process of implementing evidence-based practices in autism throughout the state, has evolved into a a collaborative effort by key agencies mandated to provide services for individuals with autism.

The Partnership for Hope: Statewide Collaboration to Change the Implementation of Home and Community Based Services in Missouri

In 2007 the Missouri Association of County Developmental Disabilities Services studied Medicaid waivers with the goal of learning about innovative HCBS implementation. This study's recommendations to the Missouri Division of DD led to the creation of the Partnership for Hope Waiver. PfH serves as a model for providing needed community services and supports for individuals with DD and their families through a unique Federal, State, and County partnership.
