The University of Tennessee?s Center on Disability and Employment administers a collaborative effort between the UCEDD, Division of Special Education, individual school systems, and disability network to demonstrate practices in transition from school to adult life.

Health is an often-overlooked but critical factor contributing to successful transition for young adults. Few opportunities are available for youth with disabilities to learn important health-management skills. This poster describes a statewide partnership resulting in a set of health materials for youth, their families, teachers and health care providers.
Keeping Her Low and Keeping Her Down: The Lived Experience of Students with Disabilities and Their Parents in Transition Planning

Lack of involvement and delayed planning resulted in poorly developed transition plans and outcomes.
