The University of Tennessee?s Center on Disability and Employment administers a collaborative effort between the UCEDD, Division of Special Education, individual school systems, and disability network to demonstrate practices in transition from school to adult life.

A Partnership Approach to Embedding Health into the Transition Process

Health is an often-overlooked but critical factor contributing to successful transition for young adults. Few opportunities are available for youth with disabilities to learn important health-management skills. This poster describes a statewide partnership resulting in a set of health materials for youth, their families, teachers and health care providers.

Keeping Her Low and Keeping Her Down: The Lived Experience of Students with Disabilities and Their Parents in Transition Planning

This session discusses the findings and recommendations from a study of the experiences of transition aged students and their parents as they navigate the transition planning process. Lack of involvement and delayed planning resulted in poorly developed transition plans and outcomes.
