The Facilitators Forum provides education in person-centered planning, a network for ongoing support for facilitators, and a web-based matching service for individuals and facilitators. The long term goal is to provide state-wide access to person-centered planning to build natural supports in community settings for adults and children with developmental disabilities.
Missouri Developmental Resource Center and Resource Folder: An Example of State-wide Collaboration and Systems Change for Individuals and Families

This session demonstrates the evolving MODDRC role with children/ adults with disabilities and families. The role expanded from individualized information/referral, to consumer/family mentoring, leadership training, and development of a cadre of volunteers. The Resource Folder is a example of broad collaboration and an exceptional tool in effecting true systems change.
