Transition Specialist - Center for Disabilities and Development, The University of Iowa

Provide support and services to individuals transitioning from an Intermediate Care Facility/Mental Retardation program to a home or community-based living setting through a demonstration grant funded by Medicaid. Duties include team coordination, serving as initial point of contact, perform assessments and resource counseling, care coordination and processing non-typical Medicaid authorized services. Position located in Des Moines with required travel.
