Position Title: Options and Transitions Leader
Employment Classification: Full Time, Regular, non-exempt
Work Site: ILCNSCA, 27 Congress St, Suite 107, Salem, MA 01970
General Statement of Duties:
Assist the Agency Management Team with insuring that all contract deliverables are met, with particular focus on nursing home transitions as well as remaining in the community. Provide monitoring of multiple contracts deliverables and develop reports as assigned. Provide deliverables as assigned for outreach and community education. Provide Information and Referral, individual and community services, with focus on managing all staff competencies and skills on Nursing Home Transitions and Money Follows the Person initiatives. Assist Management Team with referral management in WILD and data entry oversight in WILD, reports, as well as to SHINE resources and other Virtual Gateway resources. Assist with provision of ACA initiatives and ADRCGNS, Inc. programs, such as Duals Initiative, Money Follows the Person, Options Counseling Program. Promote and model Independent Living philosophy as well as consumer control and consumer directed tenets.

Supervision Exercised: IL Peer Guide/Options Counselors (2);
General Statement of Duties and Responsibilities as assigned by Supervisor:
- Assist Deputy Director with coordination and matching of consumer assignments for services,
deliverables tracking, and reporting. - Coordinate monitoring of all transition (from nursing homes; child/adolescent to adult services;
transition to employment) service provision and reporting within Agency as well as billing and
monitoring reports for funders. - Coordinate monitoring of all Options Counseling Program services provision and reporting within
Agency as well as billing and monitoring reports for funders. - Coordinate for agency all billing, reporting, monitoring of IL Program and VR IL Contract services
of supervisees, as well as Agency reporting and billing of MFP, HSE, and Duals services and
other initiatives as they are assigned. - Insure all procedures for Direct Services programs of Options, Transition, MFP including HSE and
TE are up to date and provide support to all direct services staff in executing those procedures. - As per contracts and Agency plan provide core services as follows to consumer assigned:
• Provide Information & Referral services;
• Provide intake and assessment of eligibility;
• Provide peer counseling and skills training services;
• Provide individual and community advocacy services; and service coordination services to
assigned consumers, especially to those looking to leave nursing homes and return to the
community as well as to remain in the community.
7. Maintain accurate Consumer Service Records (CSR) both paper and web based following all
ILCNSCA policies, procedures, and standards.
8. Develop and periodically review an Independent Living Service Plan with each consumer who
has a current CSR.
9. Insure accurate CSR and services provision data collection and daily data entry to all required
data systems by self and supervisees.
10. Provide supervision to assigned staff with support from Deputy Director and Executive Director,
following ILCNSCA protocols.
11. Extend the Agency's visibility and increase the interaction between the Agency and 25
communities in our service area by coordinating outreach and community education assignments
in line with Agency priorities.
12. Prepare monthly, quarterly, and annual reports as required.
13. Provide Community Education and Outreach services along with collaboration and networking for
Agency.
14. Represent Agency at meetings, with focus on transitions, ACA initiatives, and ADRCGNS, Inc.
partnerships.
15. Seek supervision and training as needed.
Qualifications:
1. Must be committed to Independent Living Philosophy and the IL paradigm.
2. Bachelor's Degree in Social Work, Psychology, Counseling or Related Field or comparable Life
Experiences.
3. MA Certificate of Completion of Options Counseling Training, or able to obtain such within 6
months of employment.
4. Person with disability and/or with skills necessary to establish effective peer relationships.
5. Three to five years' experience in peer counseling, information and referral, social work,
psychology, counseling or related field and experience in coordinating services for one or more of
the target populations as well as experience supervising other staff, paid and volunteer.
6. Must be responsible for own transportation.
7. Computer literate in all Microsoft Office applications, including MS Word, web based database
information entry and report development, Excel, and Outlook as well as web/social media
experience and competency.
8. Strategic thinking, follow through and problem solving skills.
9. Excellent customer relations skills.
10. Good knowledge of procedures and practices relating to confidentiality, benefits and services
available to older adults and persons with disabilities, and independent living.
11. Active listening, clear writing and advocacy skills.
12. Knowledge of the long term care and social service delivery systems, community resources and
the local service systems.
13. Knowledge of human behavior, family dynamics, interpersonal relations, social interaction and
social problem-solving options.
14. Knowledge and commitment to civil rights advocacy, disability rights preferred.
15. Ability to organize and plan work effectively and is well organized and demonstrates ability to
multi-task
16. Ability to communicate clearly and effectively, orally and in writing, in English.
17. Ability to network and maintain positive, effective relationships with the community agencies,
providers, families, consumers and the general public.
18. Ability to be flexible and work in a fast-paced environment.
19. Ability to work constructively as a team member.
20. Personal experience with successfully living independently with disabilities a plus.
21. Advocacy and self-advocacy skills required.
22. Bi-lingual English/Spanish is desired to provide services to customers who speak English and/or
Hispanic languages.
Salary: Salaried at $32,000-$34,000 post-introductory period, during Introductory Period, $17/hour
Hours: 35 hours per week minimum.
Benefits: As stated in agency Employee Manual.
Review Policy: Supervisor will complete performance appraisal with employee at the completion of a three-month introductory period. Job duties and assignments will be reviewed at this time. Thereafter, performance appraisals will be completed and reviewed annually.
