Functions/Duties of Position 
Position Description:
The appointee shall provide services as assigned by the supervisor in furtherance of the university's missions and goals of teaching, research, patient care, outreach and public service.

The duties of this position include:


Clinical Service/Education (50%):  Conducts mental health and psychosocial assessments with patients and their' families as part of a multidisciplinary team member and/or part of the social work clinic providing evaluation and diagnosis. The Social Worker will identify and assess the mental health, psychosocial and cultural conditions of patients and their families in regards to patient functioning capacity and support systems available for treatment planning. Participates in treatment team meetings with various health care staff to provide consultation and/or psychosocial assessments that are used in establishing treatment goals, resources and referrals. As appropriate makes selective visits to patient homes, schools, and social service agencies.

Refers patients/family to community resources for assistance (financial, vocational, schools, transportation, insurance, housing, medication) and assists family in gaining access to resources. Develops treatment plans emphasizing continuity of care, appropriate placement and maximum use of appropriate resources. Advocates for the patient within the medical setting and community. Assists clients and families in obtaining and maintaining health insurance. Provides assistance in resolving issues with health insurance payment and coverage.

Provides crisis, short and long term counseling to patients and/or their families. Provides supportive counseling to reinforce patients' strengths and assist them in dealing with the stress and feelings of grief, fear, and anger associated with acute and chronic illness, injury, hospitalization, or disability. May facilitate group therapy/sessions to provide information to patients and their families around social and emotional development.

Participates in LEND Social Work training. Assists in the development and guidance of curriculum, provides structured mentorship in interdisciplinary practice venues, and evaluates learning and performance. This effort will include all facets and stages of clinical education for students and fellows from Social Work, as well as interdisciplinary clinical education for trainees from related disciplines.

Participates in weekly reflective supervision with Social Work supervisor and group supervision with IDD Social Work & Family Support Discipline.

Departmental Functions (10%): 
The person in this position contributes to the development and evaluation of new programs to meet identified patient needs. Provides community education and support programs. Assumes active role in meetings and conferences for the purpose of information exchange, education and enhanced patient care. Prepares mental health and psychosocial assessments, histories, and treatment recommendations. Maintains written records and electronic records as appropriate of all social work involvement with patients. Prepares accurate and timely statists on assigned cases. Represents the CDRC/IDD at community programs. Attends professional continuing education seminars as required for Licensed Clinical Social Work status. Reviews professional literature to update knowledge base.

Research (15%):  The person in this position engages in research appropriate to Social Work and cooperates in joint research projects with other faculty and LEND trainees. The incumbent will be expected to contribute to population-based research and evidence based research, to generate grant-funded activities, and to disseminate the products of the research through professional publications and presentations.

Professional/Academic Activities (15%): 
The person in this position participates in other activities appropriate to the discipline, and consistent with the mission and goals of the CDRC/IDD and OHSU. Such activities include, but are not limited to, consultation and technical assistance to community agencies and providers, serving on agency and university committees, and participation in departmental meetings and in-service presentations.

Job Requirements
Position Conditions/Qualifications:
The CDRC Licensed Clinical Social Worker must hold a graduate degree in Social Work. They will have a minimum of 3 years clinical experience working with mental health/disabilities and children and families. Preference is given to candidates with bilingual language experience. Must hold current clinical social work licensure in the State of Oregon.
