"Supporting the Health Care Transition from Adolescence to Adulthood in the Medical Home," a clinical report by the AAP, the American Academy of Family Physicians and the American College of Physicians, appears in the July issue of Pediatrics. An algorithm walks physicians through the transition process for all youth in their practice, including those with chronic conditions, to the care of an adult medical home.
